Why are my legs swollen?

Leg swelling can have many possible causes, including:

  • Venous insufficiency
  • Lymphoedema
  • Lipoedema
  • Fluid retention
  • Medication side effects
  • Heart, kidney or other medical conditions

Assessment is important because treatment depends on the underlying cause.

Lymphoedema is swelling related to impaired lymphatic drainage. It may affect one or both limbs and often involves the feet.

Lipoedema is a chronic condition involving abnormal fat distribution, predominantly affecting women and commonly involving both legs symmetrically. Symptoms may include heaviness, tenderness, pain and easy bruising. In many cases, the feet are relatively spared until later stages where secondary lymphatic dysfunction (sometimes called lipolymphoedema) may develop.

Some patients may also have overlapping venous disease, lymphatic dysfunction and lipoedema, which is why careful assessment is important.

Compression garments may help reduce symptoms such as swelling, heaviness and discomfort in selected patients.

The most appropriate compression type and strength depends on the underlying condition and individual circumstances.

Medical assessment is important if swelling is:

  • Persistent
  • One-sided
  • Painful
  • Associated with skin changes
  • Rapidly worsening
  • Associated with shortness of breath or other concerning symptoms
